Dr. Lee, it depends on the sites, for both girls and the videos. I've seen posh girls doing it, on a few sites. They made a few thousands of dollars a month, so they could be quite poshy. :)
The video depends a lot on the cameras. If the models use regular camcorders, with good lighting, the video can be of very high quality. But most usually use some cheap webcams, hence the bad quality. Some sites already use high definition video, user-controlled cameras, etc. Things are evolving.
